Eligibility Requirements:
I acknowledge that I have read and fulfill the eligibility requirements and all the information provided on this questionnaire is true and accurate and any false or misleading information submitted herein is grounds for my immediate elimination from consideration. *

In order to be eligible to be selected and participate in the “Untitled Bachelor Senior Dating Show” (“Program”), applicants must meet the following eligibility requirements: 1. All Applicants must be legal residents of the United States or Canada (excluding Quebec). 2. Applicants may not presently be a candidate for any type of elected political office ("Candidate") and may not become a Candidate from the time the application is submitted until at least one (1) year after initial broadcast of the last episode of the Program in which the applicant appears, if selected. 3. All applicants must have a passport valid for one (1) year following the submission of the application and must be able to obtain any visas and/or documentation required to: a) travel without restrictions to and from the United States and b) travel without restrictions to and within the European Union. 4. All applicants must be single. To qualify as "single," the applicant must not currently be involved in a committed intimate relationship, which includes: any marital relationship (whether or not the parties are separated or currently in the process of divorcing or annulling such marriage); any co-habitation relationship involving physical intimacy; or a monogamous dating relationship more than two (2) months in duration. 5. Applicants must never have been convicted of a felony or ever had a restraining order entered against them. 6. Neither the applicant, nor any member of applicant’s immediate family (parents, siblings, children) may be an employee, officer or director of the following entities: Warner Media, LLC, Next Entertainment, Inc., NZK Productions Inc. (“Producer”), American Broadcasting Companies, Inc., known major sponsors of the Program, any station initially airing the Program, any entity associated or affiliated with the Executive Producers of the Program and/or their respective parents, subsidiary and affiliated entities. Additional Requirements: If selected for the Program, you will be required to make the following representations and warranties, and agree and acknowledge that Producer will have the following rights with respect to the materials submitted by you: 1. Each applicant acknowledges, understands and agrees that Producer may disclose any information contained within or derived from his or her application to third persons connected with the Program and to compile information from third parties in connection with such application and the application process about applicant's private, personal and public life, personal relationships with third persons, confidences and secrets with family, friends, significant others, including without limitation: physical appearance; personal characteristics/habits (both physical and mental); medical treatment/history (both physical and mental); sexual history; educational and employment history; military history; criminal investigations, charges and records; personal views and opinions about life, the world, politics, religion, and the like (collectively, "Personal Information"). Applicant acknowledges and agrees that Producer may reveal such Personal Information to third parties in the course of the application process and/or the Program and that Applicant releases, discharges, and holds harmless Companies (as defined below) from any and all claims and damages arising from such compilation or disclosure of Personal Information. 2. Each applicant acknowledges, understands and agrees that he or she, if selected for the Program, may be audio and/or videotaped up to twenty-four (24) hours a day, seven (7) days a week while participating in the Program (collectively, "Recordings") and that such Recordings will be owned by Producer. 3. Applicants acknowledge, understand, and agree that Companies’ (as defined below) use or revelation of Personal Information and Recordings as defined in these Eligibility Requirements and Representations may be embarrassing, unfavorable, humiliating, and/or derogatory and/or may portray him or her in a false light. Each applicant agrees to release, discharge and hold harmless the Companies from any and all claims (including, without limitation, claims for slander, libel, defamation, violation of rights of privacy, publicity, personality, and/or civil rights, depiction in a false light, intentional or negligent infliction of emotional distress, copyright infringement, and/or any other tort and/or damages arising from or in any way relating to the submission of an application, participation in the selection process, participation in the Program, the use of the Personal Information or Recordings and/or the use of the applicant's name, voice, and/or likeness in connection with the Program, or the promotion thereof in all media now known or hereafter devised. Applicants are required to sign Releases to this effect. 4. "Companies" as used herein shall collectively mean NZK Productions, Inc., Next Entertainment, Warner Horizon Alternative Television Inc., all other subsidiaries and divisions of Warner Bros. Entertainment Inc., Warner Media LLC, American Broadcasting Companies, Inc., Program broadcaster and distributors and all other companies associated with the production, administration and/or exhibition of the Program and/or any of their respective licensees, assigns, parents, affiliated and subsidiary companies, divisions and joint venture partners, and each of their respective employees, officers, directors and agents. 5. All applicants must authorize Producer to conduct a background check. 6. All applicants who are selected to proceed in the casting process for the Program must be available to travel to Los Angeles for further interviews. (Location and date to be determined and are subject to change in Producer's sole discretion). 7. All applicants who are selected to travel to Los Angeles may be required to undergo physical, medical and psychological examinations and testing (to be conducted in Los Angeles by qualified personnel selected by the Producer) and meet all physical, medical and psychological requirements, as set by Producer in its sole discretion. 8. Applicants who are invited to attend a casting session shall treat all information and materials received or acquired about Producer, Companies, the selection process and/or the Program, as strictly confidential and shall not disclose or otherwise disseminate any such information to any third party. Such applicants must sign a Confidentiality Agreement in the form presented by Producer. 9. Upon Producer's request, all applicants chosen to participate as a contestant or an alternate in the Program (in Producer’s sole discretion) must agree to sign any additional releases or authorizations that Producer, in its sole discretion, deems necessary. 10. Applicants who are chosen to participate as a contestant or an alternate in the Program (in Producer’s sole discretion) must be available to travel and participate in the Program for selected days over a six (6) month period for one (1) year following the submission of the application, and to participate in taping additional Program materials and in promotional activities for the Program for selected days thereafter upon Producer's request. (Dates and/or location to be determined and subject to change in Producer's sole discretion). 11. Applicants understand and agree that the Eligibility Requirements and Additional Requirements may be changed, modified, or amended by Producer in its sole discretion from time to time.

This information may include for example, contact details (e.g., your name, address, email address and/or phone number) or casting information (e.g., gender, personal performance (including an image or video of you)) and details about your experience (e.g., portfolio or showreel etc.). In certain circumstances, in particular if you are applying for one of our Productions, this information may also include your interests, audition materials and casting notes as well as protected categories of personal information such as information about your sex life/sexual orientation, medical information, political opinions, religious or philosophical beliefs, trade union membership, genetic/biometric data and criminal history data. If we do not ask you to provide this personal information as part of our application process, please refrain from providing such information (e.g., in your portfolio or showreel etc.). We may have access to certain information about your location, such as your country or address, when you provide it or via device information (such as an IP address). If you access our websites or apps on your mobile device, we may collect information about your device’s precise location. We will tell you where information collection is mandatory – this will include information that is necessary to perform any contract we may enter into with you, or indicated as mandatory on an application form. Failure to provide information may be a breach of your agreement with us. Where information is optionally provided, for example details of an emergency contact, there may be some adverse consequences if you refuse to provide this. When your application is successful, the information we collect about you will include your inclusion in our footage. If you do not apply, but otherwise feature in one of our Productions (for example, if you feature in the background of one of our films or participate in user-generated content submitted by another individual) then we will only process information that appears in our footage, or that you otherwise supply to us on any relevant release form or in any communication you send to us. HOW WE USE THE INFORMATION We use information for the purposes described in this Privacy Policy and as further disclosed to you. IF APPLICABLE LAW DOES NOT ALLOW ALL OR ANY PART OF THE ABOVE LIMITATION OF LIABILITY TO APPLY TO YOU, THE LIMITATIONS WILL APPLY TO YOU ONLY TO THE EXTENT PERMITTED BY APPLICABLE LAW. TO THE EXTENT PERMITED BY APPLICABLE LAW, ANY DISPUTE, CLAIM OR CONTROVERSY ARISING OUT OF OR RELATING IN ANY WAY TO THE SERVICE OR YOUR USE OF THE SERVICE, THESE TERMS OF USE, OR THE RELATIONSHIP BETWEEN US, MUST BE COMMENCED WITHIN ONE YEAR OF THE RELEVANT EVENTS. A DISPUTE IS COMMENCED IF IT IS FILED IN AN ARBITRATION OR, IF THE DISPUTE IS NON-ARBITRABLE, A COURT WITH JURISDICTION, DURING THE ONE-YEAR PERIOD. IF YOU OR WE PROVIDE NOTICE OF A DISPUTE UNDER SECTION 16 (DISPUTE RESOLUTION), THE ONE-YEAR PERIOD IS TOLLED FOR 60 DAYS FOLLOWING RECEIPT OF THE NOTICE OF DISPUTE. YOU AND WE EACH WAIVE—THAT IS, GIVE UP—THE RIGHT TO PURSUE ANY DISPUTE, CLAIM OR CONTROVERSY THAT IS NOT FILED WITHIN ONE YEAR AND ANY RIGHT YOU OR WE MAY HAVE HAD TO PURSUE THAT DISPUTE, CLAIM OR CONTROVERSY IN ANY FORUM IS PERMANENTLY BARRED. Indemnity You agree to indemnify and hold harmless Warner and its directors, officers, shareholders, parents, subsidiaries, affiliates, partners, agents, and licensors (collectively, the "Indemnified Parties") from and against all losses, expenses, damages and costs, including reasonable attorney fees and costs, resulting from: (i) your breach of any of the representations, warranties, and agreements made hereunder; (ii) your use of the Service; (iii) your placement or transmission of any User Content onto the Service; (iv) any use of your Account in violation of this Agreement or your failure to fulfill any obligations incurred through the use of your Account by you or a third party; or (v) your willful misconduct. Dispute Resolution Summary: Our customer-service department can resolve most customer concerns quickly and to the customer’s satisfaction. Please contact us via email to legal@wb.com so that we may attempt to resolve the dispute informally. In the unlikely event that you're not satisfied with customer service's solution (or if Warner has not been able to resolve a dispute it has with you after attempting to do so informally), we each agree to resolve those disputes through binding arbitration or small claims court instead of in courts of general jurisdiction. Arbitration is more informal than a lawsuit in court. Arbitration uses a neutral arbitrator instead of a judge or jury, allows for more limited discovery than in court, and is subject to very limited review by courts. Unless expressly limited by this Dispute Resolution provision, arbitrators can award the same damages and relief that a court can award. Any arbitration under this Agreement will take place on an individual basis; class arbitrations and class actions are not permitted. For any non-frivolous claim that does not exceed $75,000, we will pay all costs of the arbitration. Moreover, in arbitration you are entitled to recover attorneys’ fees from us to at least the same extent as you would be in court. In addition, under certain circumstances (as explained below), we will pay you more than the amount of the arbitrator’s award if the arbitrator awards you an amount that is greater than what we have offered you to settle the dispute. ARBITRATION AGREEMENT Claims Subject to Arbitration: Warner and you agree to arbitrate all disputes and claims between us, except for claims arising from bodily injury or that pertain to enforcing, protecting, or the validity of your or our intellectual property rights (or the intellectual property rights of any of our licensors, affiliates and partners). This agreement to arbitrate is intended to be broadly interpreted. It includes, but is not limited to: claims arising out of or relating to any aspect of the relationship between us, whether based in contract, tort, fraud, misrepresentation or any other statutory or common-law legal theory; claims that arose before this or any prior Agreement (including, but not limited to, claims relating to advertising); claims for mental or emotional distress or injury not arising out of physical bodily injury; claims that are currently the subject of purported class action litigation in which you are not a member of a certified class; and claims that may arise after the termination of this Agreement. References to "Warner," "you," and "us" include our respective subsidiaries, affiliates, agents, employees, licensees, licensors, and providers of content as of the time your or our claim arises; our respective predecessors in interest, successors, and assigns (including AT&T and its affiliates); and all authorized or unauthorized users or beneficiaries of Service under this or prior Agreements between us. Notwithstanding the foregoing, either party may bring an action in small claims court seeking only individualized relief, so long as the action remains in that court and is not removed or appealed to a court of general jurisdiction. This arbitration agreement does not preclude you from bringing issues to the attention of federal, state, or local agencies. Such agencies can, if the law allows, seek relief against us on your behalf. You agree that, by entering into this Agreement, you and we are each waiving the right to a trial by jury or to participate in a class action. This Agreement evidences a transaction in interstate commerce, and thus the Federal Arbitration Act governs the interpretation and enforcement of this provision. This arbitration provision shall survive termination of this Agreement. Pre-Arbitration Notice of Disputes: A party who intends to seek arbitration must first send to the other a written Notice of Dispute ("Notice"). The Notice to Warner should be sent by certified mail to: General Counsel, Warner Bros. Entertainment Inc., 4000 Warner Blvd., Burbank, CA 91522("Notice Address"). The Notice must (a) describe the nature and basis of the claim or dispute; and (b) set forth the specific relief sought ("Demand"). If we and you do not reach an agreement to resolve the claim within 30 days after the Notice is received, you or we may commence an arbitration proceeding. During the arbitration, the amount of any settlement offer made by us or you shall not be disclosed to the arbitrator until after the arbitrator determines the amount, if any, to which you or us is entitled. You may download a form to initiate arbitration at: adr.org/sites/default/files/Consumer_Demand_for_Arbitration_Form_1.pdf Arbitration Procedure: The arbitration will be governed by the Consumer Arbitration Rules (“AAA Rules”) of the American Arbitration Association (“AAA”), as modified by this arbitration provision, and will be administered by the AAA. (If the AAA is unavailable, another arbitration provider shall be selected by the parties or by the court.) The AAA Rules are available online at www.adr.org, by calling the AAA at 1-800-778-7879, or by requesting them in writing at the Notice Address. All issues are for the arbitrator to decide, except that issues relating to the scope and enforceability of the arbitration provision or whether a dispute can or must be brought in arbitration are for the court to decide. The arbitrator may consider but shall not be bound by rulings in other arbitrations involving different customers. Unless we and you agree otherwise, any arbitration hearings will take place in the county (or parish) of your billing address. If your claim is for $10,000 or less, we agree that you may choose whether the arbitration will be conducted solely on the basis of documents submitted to the arbitrator, through a telephonic hearing, or by an in-person hearing as established by the AAA Rules. If your claim exceeds $10,000, the right to a hearing will be determined by the AAA Rules. Regardless of the manner in which the arbitration is conducted, the arbitrator shall issue a reasoned written decision sufficient to explain the essential findings and conclusions on which the award is based. Except as provided in subsection (F) below, the arbitrator can award the same damages and individualized relief that a court can award under applicable law. Arbitration Fees: After we receive notice at the Notice Address that you have commenced arbitration, we will promptly reimburse you for your payment of the filing fee, unless your claim is for greater than $75,000 in value. (The filing fee currently is $200 but is subject to change by the arbitration provider. If you are unable to pay this fee, we will pay it directly upon receiving a written request at the Notice Address.) We will pay all AAA filing, administration, and arbitrator fees for any arbitration initiated in accordance with the notice requirements above. If, however, the arbitrator finds that either the substance of your claim or the relief sought in the Demand is frivolous or brought for an improper purpose (as measured by the standards set forth in Federal Rule of Civil Procedure 11(b)), then the payment of all such fees will be governed by the AAA Rules. In such case, you agree to reimburse us for all monies we previously paid that are otherwise your obligation to pay under the AAA Rules. In addition, if you initiate an arbitration in which you seek relief valued at greater than $75,000 (either to you or to us), the payment of these fees will be governed by the AAA rules. Alternative Payment: If you initiated arbitration in accordance with the notice requirements above in subsection (B) and the arbitrator issues an award in your favor that is greater than the value of our last written settlement offer made before an arbitrator was selected, then we will pay you the amount of the award or $10,000 (“the alternative payment”), whichever is greater. If we did not make a written offer to settle the dispute before an arbitrator was selected, you will be entitled to receive the alternative payment if the arbitrator awards you any relief on the merits. The arbitrator may make rulings and resolve disputes as to the payment and reimbursement of fees, expenses, and the alternative payment at any time during the proceeding and upon request from either party made within 14 days of the arbitrator’s ruling on the merits. In assessing whether an award that includes attorneys' fees or expenses is greater than the value of our last written settlement offer, the calculation shall include only the portion of the award representing attorneys' fees or expenses that you reasonably incurred pursuing the arbitration through the date of our settlement offer. Although under some laws we may have a right to an award of attorneys’ fees and expenses if we prevail in an arbitration, we agree that we will not seek such an award. Requirement of Individual Arbitration: The arbitrator may award declaratory or injunctive relief only in favor of the individual party seeking relief and only to the extent necessary to provide relief warranted by that party’s individual claim. YOU AND WE AGREE THAT EACH MAY BRING CLAIMS AGAINST THE OTHER ONLY IN YOUR OR OUR INDIVIDUAL CAPACITY, AND NOT AS A PLAINTIFF OR CLASS MEMBER IN ANY PURPORTED CLASS, REPRESENTATIVE, OR PRIVATE ATTORNEY GENERAL PROCEEDING. Further, unless both you and we agree otherwise, the arbitrator may not consolidate more than one person’s claims and may not otherwise preside over any form of a representative, class, or private attorney general proceeding. If, after exhaustion of all appeals, any of these prohibitions on non-individualized declaratory or injunctive relief; class, representative, and private attorney general claims; and consolidation are found to be unenforceable with respect to a particular claim or with respect to a particular request for relief (such as a request for injunctive relief sought with respect to a particular claim), then that claim or request for relief shall be severed , and all other claims and requests for relief shall be arbitrated. Future Changes to Arbitration Provision: Notwithstanding any provision in this Agreement to the contrary, we agree that if we make any future change to this arbitration provision (other than a change to the Notice Address) following your use of this Service, you may reject any such change by sending us written notice within 30 days of the change to the arbitration Notice Address provided above. By rejecting any future change, you are agreeing that you will arbitrate any dispute between us in accordance with the language of this provision. General Terms Governing Law and Venue This Agreement shall be governed by, construed, and enforced in accordance with the laws of the State of California, as they are applied to agreements entered into and to be performed entirely within California and without regard to conflict of law principles, except to the extent that law is inconsistent with or preempted by federal law. To the extent that a dispute is not subject to arbitration under Section 16 (Dispute Resolution) of this Agreement, that action shall be brought in the appropriate state or federal court located in Los Angeles County, California; and we both irrevocably consent to the exclusive jurisdiction and venue of the state or federal courts in Los Angeles County, California for the adjudication of all non-arbitral claims. Force Majeure Warner will not have any liability to you by reason of any delay or failure to perform any obligation hereunder if the delay or failure to perform is occasioned by force majeure, which refers to any act of God, storm, fire, casualty, unanticipated work stoppage, power outage, satellite failure, strike, lockout, labor dispute, civil disturbance, riot, war, national emergency, Governmental action, or other cause beyond its control. No Waiver No failure or delay by Warner in exercising its rights under this Agreement will constitute a waiver of those rights, nor will any partial assertion of any such rights preclude further assertion of the same. Severability Except as specified in Section 16 (Dispute Resolution), if any provision of this Agreement shall be unlawful, void, or for any reason unenforceable, then that provision shall be deemed severable from this Agreement and shall not affect the validity and enforceability of any remaining provisions. Construction The titles of the sections of this Agreement are for convenience of reference only and are not to be considered in construing this Agreement. Unless the context of this Agreement clearly requires otherwise: (a) references to the plural include the singular, the singular the plural, and the part the whole; (b) “or” has the inclusive meaning frequently identified with the phrase “and/or;” (c) ”including” has the inclusive meaning frequently identified with the phrase “including but not limited to” or “including without limitation;” and (e) references to “hereunder,” “herein,” or “hereof” relate to this Agreement as a whole. Any reference in this Agreement to any statute, rule, regulation, or agreement, including this Agreement, will be deemed to include such statute, rule, regulation, or agreement as it may be modified, varied, amended, or supplemented from time to time. Survival Any provision herein which by its nature contemplates your continued observance following termination of this Agreement will survive termination of this Agreement. Entire Agreement This Agreement, including the Privacy Policy and any applicable Additional Terms, is the entire agreement between the parties relating to the matters contained herein. Copyright Agent If you believe that any User Content or other material on the Service infringes your copyright rights, please forward the following information in writing to our Copyright Agent at the address listed below, in conformance with the Digital Millennium Copyright Act of 1998 (“DMCA”): Your name, address, telephone number, and (if available) email address; A description of the copyrighted work that you claim has been infringed; The exact URL or a description of each place where alleged infringing material is located; A statement by you that you have a good faith belief that the disputed use has not been authorized by you, your agent, or the law; Your electronic or physical signature or the electronic or physical signature of the person authorized to act on your behalf; and A statement by you that the information in your notice is accurate and, under penalty of perjury, that you are authorized to act on behalf of the owner of an exclusive right that is allegedly infringed. CONFIDENTIALITY AND NONDISCLOSURE AGREEMENT THIS CONFIDENTIALITY AND NONDISCLOSURE AGREEMENT (“NDA”) is made and entered into by and between NZK Productions Inc. and/or its affiliated or related companies (“Company”), and (“Individual”). For good and valuable consideration, the receipt and sufficiency of which are hereby acknowledged, Individual hereby agrees as follows: CONFIDENTIAL INFORMATION; NONDISCLOSURE a. Individual is currently being considered as a contestant for the television series presently entitled “The Bachelor Senior Show” (hereinafter the “Series”). During his/her consideration, Individual may acquire or may be exposed to information (including, without limitation, information that is written, oral, photographed or recorded on film, tape, or otherwise) concerning the personal and/or business affairs of the Series, Company, and/or its officers, directors, principals, shareholders, affiliates, employees, contractors, executives, customers and clients, parent and any other related or affiliated entities (hereinafter collectively, “Company’s Affiliates”), and/or its (their) on-air talent and host(s), if any (collectively, “Host”). Individual agrees he/she shall not, both during and after his or her consideration, in perpetuity, disclose or cause to be disclosed (or confirm or deny the veracity of) to any third party, or use or authorize any third party to use: (1) any information which Company has not revealed to the general public concerning the Series, including but not limited to, the participants in the Series, the content of the Series and the outcome of the Series including any of its episodes; and (2) any non-public information relating to the business or interests of Host, Company, or Company’s Affiliates; and (3) any information developed by or disclosed to Individual by Host, Company, Company’s Affiliates, or by any third party, which is confidential to Host, Company, Company’s Affiliates, or any third party and/or which is not generally known to the general public; and (4) any information which Host, Company, or Company’s Affiliates instruct Individual not to disclose or confirm; and (5) any information relating to the public or private lives of Host, Company, or Company’s Affiliates (whether or not taken, created or obtained by Individual), personal telephone numbers and addresses of residence(s), personal affairs and relationships, the names and addresses of personal friends, personal and business schedule and appointments and the locations thereof, and professional or personal activities. Confidential Information is and shall remain the sole and exclusive property of Company and, during and after Individual’s consideration, Confidential Information, when entrusted to Individual’s custody, shall be deemed to remain at all times in the sole possession and control of Company. Notwithstanding the foregoing, Individual may disclose such Information pursuant to subpoena or other legal process five (5) business days after giving prior written notice to Company, which notice shall specify the Confidential Information to be disclosed and the circumstances of such disclosure. b. Without limiting any other provision hereof, Individual shall not give any interviews regarding or otherwise participate in the disclosure of any Confidential Information or any other information relating to this NDA, Individual’s consideration or the business of Company or Company’s Affiliates without the prior written consent of Company. In the event that Individual is contacted by a journalist, a representative of the media or other third party who requests that Individual disclose or confirm or deny the veracity of any of the information covered by this NDA, Individual shall reject said request and/or issue a “no comment,” and Individual shall immediately advise Company thereof. c. Within forty-eight (48) hours of the end of Individual’s consideration for any reason whatsoever, or whenever required by Company, Individual shall return, or cause to be returned to Company, all of the Confidential Information (including, without limitation, all copies of scripts, readers’ reports, summaries of contracts, blueprints, designs, drawings, and financial information) or any other property of Company in Individual’s possession or custody or at his or her disposal, which he or she has obtained or been furnished, without retaining any copies thereof. d. Individual agrees that any breach of this NDA will cause Company incalculable damages. Such damages include the costs associated with developing, selling, casting, producing and distributing the Series, as well as the incalculable management time necessary in accomplishing same. Accordingly, Individual agrees that in the event of breach of this NDA, Individual shall pay Company, upon demand, as liquidated damages, the sum of Twenty-Five Thousand Dollars ($25,000.00) plus any actual out-of- pocket expenses, as well as any attorney fees expended in enforcing this NDA. e. If Individual breaches the confidentiality obligations set forth in this NDA, Company shall be entitled to injunctive relief against Individual to enforce the NDA and shall be entitled to recover all attorneys’ fees and costs associated with enforcing this provision. The provisions of this NDA shall be binding upon and shall inure to the benefit of Company, its successors and assigns and to the benefit of Individual and his or her successors and assigns.
